Doula Matching Intake Form

Healthy Mothers, Healthy Babies Coalition of Georgia (HMHBGA) wants to connect Georgia to free and sliding-fee doula services. We match you with a doula from Building Perinatal Support Professionals program based on location and your preferred level of engagement. HMHBGA does not offer doula services but only provides adequate referrals to doulas who have gone through our program. Once you have accepted the referral, you will be in a direct relationship with your professional doula and release HMHBGA from any liability associated with this relationship.
